Parent/Guardian Consent, Waiver & Release Form

I, the undersigned parent or legal guardian, authorize my child to participate in the Young Yogis Summer Immersion program organized by Yoga Club PR.

I understand that activities may include, but are not limited to: * Children’s yoga and movement classes * Mindfulness and breathing exercises * Creative arts and crafts * Outdoor activities * Educational games and group activities * Walking excursions * Off-site field trips * Transportation to and from scheduled activities

I understand that while Yoga Club PR and its staff will take reasonable precautions to provide a safe environment, participation in physical activity and group programs carries inherent risks.

I authorize my child to participate in all scheduled Young Yogis Summer Immersion field trips and activities, including transportation provided or arranged by Yoga Club PR.

I give permission for staff and authorized volunteers to supervise my child during these activities.

In the event of illness, injury, or emergency, I authorize Yoga Club PR staff to obtain emergency medical treatment for my child if I cannot be reached immediately.

I understand that every reasonable effort will be made to contact me before treatment is provided. I agree to be financially responsible for any medical expenses incurred.

I acknowledge that participation in yoga, physical activity, outdoor recreation, transportation, and field trips involves inherent risks that cannot be completely eliminated. On behalf of myself and my child, I voluntarily assume all risks associated with participation in the Young Yogis Summer Immersion. To the fullest extent permitted by law, I release, waive, and discharge Yoga Club PR, its owners, instructors, employees, contractors, volunteers, and affiliates from any claims, demands, liabilities, damages, or causes of action arising from participation in the program, except in cases of gross negligence or willful misconduct.

Throughout the program, photographs and videos may be taken for educational, promotional, and marketing purposes.

YES, I grant permission for my child’s image, likeness, and participation to appear in Yoga Club PR marketing materials, website content, social media platforms, and promotional campaigns.

NO, I do not grant permission for my child’s image to be used.

I understand that all participants are expected to: * Respect instructors, staff, and fellow participants * Follow safety guidelines * Participate respectfully in activities * Refrain from behavior that may endanger themselves or others I understand that repeated unsafe or disruptive behavior may result in dismissal from the program without refund.

I certify that I am the parent or legal guardian of the child listed above. I have read this document in its entirety, understand its contents, and voluntarily agree to all terms.
