Participant Waiver & Release of Liability

I acknowledge that I am voluntarily participating in yoga, fitness, wellness, and movement-based classes, workshops, or sessions offered by Yoga Club PR, either in-studio, outdoors, virtually, or at any other affiliated location. I understand these activities involve physical exertion that may be strenuous and could cause physical injury, and I assume full responsibility for any and all risks, known or unknown.

I affirm that I am in good physical condition and do not suffer from any condition or disability that would prevent or limit my participation in physical activities. I have consulted with a physician or voluntarily assume all risks if I have chosen not to do so. I understand that it is my responsibility to notify the instructor of any injuries, conditions, or limitations prior to class.

In consideration of being permitted to participate in Yoga Club PR's offerings, I knowingly, voluntarily, and expressly waive any claim I may have against Yoga Club PR, its owners, instructors, staff, affiliates, and landlords for injury, illness, accident, loss, or damages arising from my participation. This includes, but is not limited to, injuries or conditions resulting from physical activity, studio equipment, facilities, or the actions or negligence of other participants.

I agree not to sue or bring any legal action against Yoga Club PR, its owners, instructors, or affiliated parties for any claim arising out of or related to my participation in classes, programs, or activities.

I consent to the use of photographs, videos, or recordings taken during classes or events for promotional purposes by Yoga Club PR. I understand these images may be used on the Studio's website, social media, or marketing materials.

I acknowledge the potential risks of exposure to communicable diseases in group settings and voluntarily assume such risks by attending in-person classes. I release Yoga Club PR from liability for any exposure or illness, and I agree to follow all health and safety protocols put in place by the Studio.

This agreement is binding upon me, my heirs, assigns, and legal representatives. I affirm that I am at least 18 years of age (or the legal guardian of a minor participant), have read this waiver thoroughly, and understand its contents. I agree to participate at my own risk and confirm by completing this form I voluntarily sign this release.
